The game is a direct sequel to Sniper Elite III and takes players to Italy in 1943, where they will have to complete a series of dangerous missions as OSS sniper Karl Fairbairn.

The game´s plot unfolds immediately after the events of Sniper Elite III and focuses on Karl´s new mission - to eliminate General Tobias Schmidt and other officers responsible for the attack on the transport "Orchid", sunk by a new Nazi weapon. After successfully completing the mission, Carl receives a new mission – to find and eliminate Sofia “Angel” Di Rocco, the leader of a local guerrilla resistance group.
