The player takes on the role of journalist Miles Upsher, who is controlled from the first person. With him, Miles carries a notebook, in which he writes notes during the game, and a video camera. Since Miles is an ordinary person who does not wield any weapons, the video camera is the only item that can be used.

The camera is equipped with a night vision device, without which it is simply impossible to move around a poorly lit hospital. When using a night vision device, the camera starts to discharge quickly, which forces the player to constantly search for batteries scattered everywhere.

The gameplay includes solving simple puzzles, stealthy movement, as well as moments in which you need to run away from your pursuers, overcome various obstacles, and hide in the dark, lockers, under beds and in other secluded places. In the game, the main character has no allies, apart from Martin´s father and one patient who pursues and helps you throughout the game.
